BREAST REVISION & SUPER BREAST LIFT
CASE OVERVIEW
This youthful almost sixty-year old had a previous breast surgery 20 years ago with a different surgeon. During menopause her breasts grew, leaving her feeling matronly and not sexy. She spent two years researching plastic surgeons before she was referred to Dr. Anton by her hairdresser. During her consultation she explained that she didn’t want small breasts. She liked having larger breasts, but she wanted them to be more youthful and perky. Dr. Anton performed a Bilateral Breast Revision with Exchange of Implants, and a no-drain Super Breast Lift.
PATIENT PROFILE
AGE: 60
PRIMARY CONCERNS
This patient was most concerned with her aging breasts. After menopause her breasts got larger and were sagging. She had two previous breast surgeries and desired a smaller implant. She was also unhappy with the size of her areola. “I’ve always been self-conscious about that,” she explains and wanted them smaller. She spoke to a lot of different surgeons closer to her Los Angeles home but had not met a surgeon she trusted or felt confident in redoing her breasts. Then she saw Dr. Anton’s before and after photos. “I knew I found my surgeon. His work was just drastically different from every other surgeon I spoke with.”
CONSULTATION & ASSESSMENT
Before being referred to Dr. Anton by her hairdresser, she spent two years researching plastic surgeons and met with several in the Beverly Hills and LA area near her home. She was not satisfied with the work they presented in their before and after photos and one even misdiagnosed her with capsular contracture. She had the patience to wait until she found the right surgeon. “It was obvious from looking at Dr. Anton’s photos that he is a fabulous surgeon. A perfectionist.” She explained to him that up until menopause she had really liked the size and shape of her breasts. Then they changed. “They were a little too conspicuous and just not what I wanted. I wanted something more age appropriate, but not too small.” She explained that she desired fullness in the upper breasts and more cleavage. Dr. Anton recommended a Breast Revision with Implant Exchange to a smaller implant and a Super Breast Lift to remove the excess skin and give her the “perkier, younger” breasts she desired along with an areolar reduction.

TREATMENT PLAN
– Bilateral Implant Revision
– Removal of 700 cc Mentor smooth silicone implants
– Placement of 450 cc Natrelle smooth silicone implants under the muscle
– Super Breast Lift
– Areola reduction
A Bilateral Implant Revision with Super Breast Lift is performed under general anesthesia in an outpatient surgery facility. Patients typically go home after the procedure, but can stay at the after-care facility if desired. Before surgery Dr. Anton marked the patient in the sitting position. In surgery he removed 40g of tissue from her left side to make her breasts even. He then removed her 700cc Mentor smooth gel implants and tightened the capsule to prepare them for the new, smaller implants. Various sized sizers were tried to get the size she desired. Before placing the new implants, the underlying dermis was tightened in the superior nipple-areolar, lower breast curve (creates the dermal bra) and the lower breast fold. 405cc Natrelle smooth, moderate profile SCM implants were then placed under the muscle and the patient placed in a support bra.
